Follow up visits are scheduled after initial visit is completed. The frequency of the visits are based on the patient’s condition. They may occur weekly, monthly or on an as needed basis. The follow up visits help to gain a better understanding of the problem, create actionable goals with accountability, and monitor progress.

• 60 min Integrative Health Assessment after Initial Intake with wellness roadmap for next 6 months
